Who can administer Botox and filler injections in Germany — The Local Germany
In Germany, cosmetic botulinum toxin injections can generally be administered only by licensed doctors, as botulinum toxin is a prescription medicine. Cosmetologists and alternative practitioners known as Heilpraktiker are not entitled to inject Botox, The Local Germany reports.
Dentists may use botulinum toxin only within the scope of dental practice — for example, to treat bruxism. At the same time, purely cosmetic Botox injections in the facial area generally do not fall within the field of dentistry.
Legal status of fillers
Hyaluronic acid-based fillers are not prescription medicines in Germany. Licensed doctors may legally perform such procedures, while cosmetologists may not.
The question of whether Heilpraktiker — state-authorized alternative practitioners who are not doctors — may inject fillers remains disputed among legal experts. Doctor and lecturer Sophia Wilk-Vollmann called the legal status of such procedures controversial and noted that Heilpraktiker do not have a general authorization to administer hyaluronic acid injections. Lawyer Thomas Bruggmann, by contrast, believes that licensed Heilpraktiker are generally allowed to inject such fillers because they are not prescription products.

Checking qualifications
Doctors do not need a separate state certificate for Botox procedures: a German medical license is generally sufficient. At the same time, patients are advised to find out what training a specialist has, how often they perform the specific procedure, and what experience they have.
There is no single national register of Botox injection specialists in Germany. Many regional medical chambers maintain public doctor search databases. The title Facharzt für Plastische und Ästhetische Chirurgie is a protected specialty requiring relevant training. By contrast, the designation Schönheitschirurg is a marketing term and does not in itself confirm formal qualifications.
In 2024, botulinum toxin was the most common non-surgical cosmetic procedure in Germany, with around 361,000 procedures performed. Hyaluronic acid fillers ranked second, with more than 220,000 procedures. According to the publication, Berlin courts have in recent months considered several cases involving cosmetologists suspected of administering Botox and filler injections without the necessary legal authorization. In one case, a client reportedly suffered facial paralysis and required hospitalization.
